Please note: All of these forms are required prior to loan submission. Lender may request other forms to be signed.
Borrowers’ Certification and Authorization
This form allows the broker and lender to obtain and verify 3rd party information on behalf of the client. Client certifies the authenticity of the information he/she provides.
Equal Credit Opportunity Act-
This form simply explains that the client cannot be discriminated against, as it is prohibited by federal law.
The Housing Financial Discrimination Act of 1977 Fair Lending Notice:
This is another government disclosure prohibiting discrimination. The form explains the rights that borrowers have against discrimination when applying for mortgage loans.
Privacy Policy Disclosure
Protects our clients’ confidential information, discloses what is shared as permitted by law and the option to opt-out.
FACTA Credit Score Disclosure Notice to the Home Loan Applicant
This form gives the client the right to obtain their credit scores as a result of applying for a loan. Client also certifies their identity via supplying a copy of a current photo ID.
Notice to Applicant of Right to Receive Copy of Appraisal Report
Client has the right to receive a copy of the appraisal upon receipt of payment for the report.
Servicing Disclosure Statement:
The Servicing Disclosure Statement form is a required federal disclosure form and is part of the Real Estate Settlement Procedure ACT (RESPA). This form simply explains that the loan be assigned, sold or transferred to another lender at anytime while this loan is outstanding without any change in the terms of the loan.

Mortgage Loan Origination Agreement:
This form is the standard mortgage loan agreement clients are required to sign.
It explains the nature of the relationship between borrower, lender, and mortgage broker.
Real Estate Agency Disclosure:
This is a disclosure form issued by the California Department of Real Estate. The form explains the agency relationship between the broker, the borrower and the lender.
Initial Loan Documentation Requirements
This is a list of all documentation required for loan process. Once we receive the pertinent documentation, we can proceed to package and submit the loan.
CPA Letter:
This is a sample CPA letter needed for self-employed clients. The lender will request a CPA letter to be submitted as proof of self-employment if you cannot provide a two-year business license. The tax individual signing the self-employment letter must be a licensed CPA or an IRS Enrolled Agent with an IRS license number.

Insurance Co/Agent Referrals:
For our clients’ convenience, we provide a list of several insurance companies located in the Coachella Valley, to obtain home owners policies required for the loan. If you do not have a specific insurance company in mind, you can always call any of these companies to shop for lower insurance rates for your new mortgage loan.
